ALL PARTICIPANTS must submit this form whether they are taking the ISV Group Flight or organising their own flight to Costa Rica.

This form and applicable deposits must be received by the ISV office as soon as possible, but no later than 100 days prior to your departure date. Your full airfare payment must be received in the ISV Office no later than 65 days prior to your departure date. If your full airfare payment is not received on or before 65 days prior to departure, your reserved space on the ISV Group Flight will be released.

FLIGHT OPTIONS

I would like to reserve my seat on the ISV Group Flight departing Auckland. I will submit a $125.00 NZD non-refundable deposit.

I will arrange my own flight to Costa Rica. I am participating in the Spanish Lesson Option, I understand I must arrive at the same time or prior to the arrival of participants on the ISV Group Flight. I understand I must provide the ISV Office with my personal flight information via email at least 65 days prior to my departure date.
As I am not participating in the Spanish Lesson Option I agree to arrange my own flight and will organise to arrive in Costa Rica in line with the dates supplied in the Information Package which I received along with my allocation. I understand I must provide the ISV Office with my personal flight information via email at least 65 days prior to my departure date.

Note: Flight costs and taxes are subject to change due to fluctuating fuel surcharges, etc. implemented by the airlines.
