ALL PARTICIPANTS must submit this form when taking the ISV Group Flight to Dominican Republic.

This form and applicable deposits must be received by the ISV office as soon as possible, but no later than 90 days prior to your departure date. Your full airfare payment must be received in the ISV Office no later than 65 days prior to your departure date. If your full airfare payment is not received on or before 65 days prior to departure, your reserved space on the ISV Group Flight will be released.
